The ideal candidate will have a Master's degree in Social Work (MSW) from a School of Social Work accredited by the Council on Social Work Education (CSWE) and hold certification as a Certified Advanced Social Work Case Manager (C-ASWCM) by the National Association of Social Workers. Alternatively, a minimum of three (3) years' experience within the preceding five (5) years as a social work case manager in a hospital-based setting performing medical crisis counseling and discharge planning is required. Qualifications: Master's degree in Social Work (MSW) from a CSWE-accredited School of Social Work. Certification as a Certified Advanced Social Work Case Manager (C-ASWCM) by the National Association of Social Workers OR a minimum of three (3) years' experience within the preceding five (5) years as a social work case manager in a hospital-based setting performing medical crisis counseling and discharge planning. At least 12 months of recent experience, within the last 24 months, providing evidence-based individual counseling in a mental health or health care facility. Minimum of 12 months of direct hands-on experience, within the last 24 months, with computer operations, basic word processing, data entry, and use of an automated medical record system. Prefer candidates with recent experience (within the past three years) providing integrated behavioral health services within a primary care setting. Current, full, active, and unrestricted license to practice as a Licensed Clinical Social Worker (LCSW). Key Responsibilities: Participate in Utilization Management/Case Management meetings to review and identify opportunities for improving treatment and recommend corrective actions. Provide monthly in-service training for non-healthcare-practitioner members of the clinical and administrative staff on relevant social work and case management topics. Facilitate patient referrals to ensure comprehensive care. Collaborate with MTF RN Case Manager(s) in developing and implementing the Case Management Program (CMP).
