CASEWORKER II 113 (472) Open Until Filled Salary: $37,598 to $45,819 Annually Salary is based on education and experience

Job Description: As a Forensic Intensive Case Manager, this position provides crisis prevention and management, monitoring, assessment, rehabilitative skills training, service planning and coordination, and jail diversion for adults with mental illness involved in the Criminal Justice system. The Intensive Case Managers objective is to assist clients in increasing independent functioning in the community, reducing recidivism in the criminal justice system, reduce hospitalizations, and developing individualized short term and long term recovery goals. Service coordination involves jail screenings, acting as MHMRABV liaison to the courts, accessing medical, social, educational, and other appropriate services that will help an individual achieve a quality of life and community participation acceptable to the individual. This goal is accomplished by completing ANSA Assessments, developing individualized recovery plan, performing skills training, intervening in the crisis situations, coordinating community resources, and documenting all contacts for persons served on caseload.The position also serves as a member of a mobile crisis outreach team (MCOT) designed to reach individuals at their place of residence, school and other community based safe locations as a means of diversion from jails, emergency rooms and hospitals.Duties inc. e accompanying specially trained Sheriffs officers (CIT) to individuals home to provide crisis assessments; service plan development and implementation; access to medication services; monitoring and follow-up; inter- and intra-agency coordination of support services; family education and support; and client advocacy in the community where client is located. Crisis on Call work required as agency need dictates. Education: Requires Bachelors Degree in Human Services or Social Services Experience: Requires experience working with individuals with mental illness, must be able to work independently as well as with a team. Good organizational skills are necessary. Experience working with criminal justice population helpful. Hours of Work: 8am-5pm; Monday through Friday. At times may need to have a flexible schedule to accommodate the needs of the program. Licensure/Certifications: Ability to be credentialed as Qualified Mental Health Professional. Valid Texas drivers license and are insurable by agency insurance. Continued employment is contingent upon successful completion of Adults Needs and Strengths Assessment (ANSA) certification as well as Illness, Management and Recovery (IMR) certification. Physical Requirements: Communicate effectively both verbally and written; ability to sit or stand for extended periods of time. Good computer skills are necessary; ability to pass the Preventative Management of Aggressive Behavior (PMAB) Training as well as all other physical requirements for this position.
