CASEWORKER II 197 (496) Open Until Filled Salary: $37,598 to $45,819 Annually Salary is based on education and experience

Job Description: Serves as one of the Local Intellectual and Developmental Disability Authoritys(LIDDA) representatives of the Community Living Options Information Process (CLOIP) Program to provide standardized information, explanation, and educational materials of available community living options to all individuals ages 22 and above who are residents of Brenham State Supported Living Center and their Legally Authorized Representative (LAR).Coordinates and arranges for sample community tours prior to annual meeting of persons interested in community placement.Submits results of tours, explanation of living options, and interview results with individuals, LARs, or advocates to individuals Inter-disciplinary Team.Based on assigned caseloadof BSSLC residents, attends annual planning meetings of all assigned individuals to discuss Community Living Options requires routine travel to Brenham State Supported Living Center. Ensures information is shared with the individuals designated LIDDA.Duties also include performing Preadmission Screening and Residential Review (PASRR) Evaluations to persons in nursing facility and providing crisis prevention and management, monitoring, assessment, and service planning and coordination to nursing facility residents who are eligible for PASRR IDD Service Coordination within our seven counties service area and as based on the Individual Service Plan (ISP), monthly monitoring of outcomes, and at least quarterly convening of service planning team. Negotiate and facilitate the array of services and support needed to address the individuals personal goals and desired outcomes as identified through the Service Coordination Assessment and Service Planning Team. Ensures planned services and support are implemented and appropriately documented, provides education in community living options, and will advocate and ensure consistent interactions with nursing facility and other agencies, as necessary. Provides required monthly face to face (and collateral) contact monitoring of persons on caseload and documents activities as required by Texas HHSC-IDD Services and MHMRABV. *Education: Requires Bachelors Degree in behavioral, social science, criminal justice, humanities, or education field. *Experience: Prefer experience working with individuals with IDD; must be able to work independently as well as with a team. Must be computer literate, requires excellent verbal and written communication skills, requires excellent organization skills, including the ability to meet rigid timelines and manage multiple priorities.Good organizational and time management skills are necessary.Knowledge of service coordination principles, objectives, standards, and methods; knowledge of Medicaid waiver program services and non-Medicaid waiver program services; skill in determining individual needs assessment and the ability to respond to changes required; ability to comprehend and implement program goals. *Hours of Work: Monday - Friday, 8:00 a.m. - 5:00 p.m.; some flexibility required occasionally to meet the needs of the program/person in services. *License/Certificati ns: Must successfully complete Agency and TAC require IDD service coordination training within allotted timelines as applicable.Valid Texas drivers license and must be insurable by Agencys insurance. **Physical Requirements: Ability to drive a vehicle; must have reliable transportation and a good driving record; must have a valid Texas drivers license and must be insurable by Agency insurance; ability to communicate effectively; ability to communicate effectively; ability to pass the Preventative Management of Aggressive Behavior (PMAB) Training as well as a
